エミネムがドクター・ドレーに見出され世界的スターダムを駆け上がる直前、どん底の極貧生活を送っていた1997年に書き上げられた、アルバム内で最もシリアスかつダークな楽曲である。当時、最低賃金の職すら解雇され、愛娘ヘイリーのオムツやクリスマスプレゼントすら買えない絶望感から、エミネムは大量の鎮痛剤（タイレノール）を服用して自殺未遂を起こしている。本作はその事件の直後にレコーディング（オリジナル版）されており、猟奇的なペルソナである「スリム・シェイディ」の狂気ではなく、貧困と自己嫌悪に押し潰されそうになりながらも野心を燃やす「マーシャル・マザーズ」としての痛切な本音が刻まれている。怒りと悲しみが同居するコーラス部分は、ヒップホップにおける「ハングリー精神」の最もリアルで残酷な描写の一つとして高く評価されている。
